Patient Agreement & Terms of Service

By booking an appointment at Northgate Medical Centre, you agree to the following terms and conditions. Please read them carefully before proceeding.

1. Appointments & Cancellations. We ask that you provide at least 2 hours notice if you need to cancel or reschedule your appointment. Repeated late cancellations or no-shows may result in a cancellation fee of up to $50.

2. Privacy & Confidentiality. Your personal and health information is collected and managed in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. Your information will only be used for the purposes of providing healthcare services and will not be shared without your consent, except as required by law.

3. Billing & Fees. Bulk billing is available for eligible Medicare cardholders for standard GP consultations. Additional services, extended consultations, procedures, and dental or cosmetic appointments may incur out-of-pocket costs. You will be informed of any fees prior to treatment.

4. Accuracy of Information. You confirm that the information provided in this form is accurate and complete to the best of your knowledge. Providing false or misleading information may affect your care and is not permitted.

5. Communication. By providing your email address and phone number, you consent to receiving appointment confirmations, reminders, and relevant health communications from Northgate Medical Centre.

6. Telehealth. Some consultations may be offered via telehealth. If applicable, you will be notified prior to your appointment. You are responsible for ensuring you have an appropriate environment and device for a telehealth consultation.
